We help primary care and family practices meet the behavioral healthcare needs of their patients. We do this in several ways:
Collaborative Care: Through our affiliation with Mindoula Health (www.mindoula.com), we offer a collaborative care service as a compliment to primary care.Our psychiatrists team up with Mindoula case managers to provide virtual psychiatry consultations, virtual care management, and bevioral assessments and reports for inclusion in the patients’ medical records. Patients pay $99/month for the Mindoula collaborative care service. The PCP remains the quarterback for care and treatment, and is able to better able to treat and manage behavioral health challenges.
Community Linkages: When a patient cannot be managed entirely within the primary care setting, we provide referrals and linkages to a wide range of behavioral health services and resources in the community.
Continuum of Care: As part of the management and consulting services we provide to community hospitals and senior living facilities, we help ensure that top-quality behavioral health services are available at each and every step in the continuum of care.
